To ensure the best experience possible we have provided a list of instructions to help you prepare for your visit.

We ask that you do not bring children with you to your appointment - for their safety, children cannot go into the back office. Thank you. 

Your Payment Options

Self-Pay

Payment is expected at the time of service (cash, credit card, or money order is preferred. We do accept personal checks for payments under $100). If you are using a credit card you will need to provide photo ID. 

Insurance Plans Accepted

With or without insurance, you can always come to us for your health care. If you plan on using insurance, you must bring in your insurance card and photo ID in order for us to bill your insurance. We will verify your coverage at every visit. Co-pays and deductible amounts are due at time of service.

Sliding Fee Scale

Health center locations in Elkhart, Gary, Michigan City, New Albany, and Seymour and receive federal funding (Title X) that allows additional discounts for patients seeking family planning services. This discount is calculated based on the total household income and family size. Health center staff will discuss fees and discounts available to you at the time of service.

In order to obtain services at the locations listed above, you must be enrolled as a patient, which means you need to have an annual exam at a PPINK health center. If you need only partial services like STI testing or treatment, please be aware you may need to have a complete exam at the time of your visit.
