How to Sign Up for the Patient Portal

Using your temporary login and password (existing patients only)

If you already have a permanent portal login, please start here instead. 

Step 1 – Review the invitation email that you received with a temporary username and password.

Step 2 – Follow the directions in the invitation email:  

  1. Click the link in the email or go to http://login.my-appointment.org
  2. Click the button “I AM NEW HERE”
     

Step 3 – Read the Terms and Conditions and accept them.
 

Step 4 – Login using your temporary username and password:

  • Enter your temporary username (all numbers)
  • Enter your temporary password, following the detailed instructions in the email for how to build your temporary password:
  • For example, if your name is John Doe and your birthday is January 5, 1975, then your temporary password is: JD01051975.

 
Step 5 – Enter the favorite color that was included in the introduction email.
 

Step 6 – Create a Username and Password


Step 7 – Login to the Patient Portal
 

Step 8 - Use the portal to make schedule or cancel appointments, communciate securely with health center staff, view lab results and more.
