Please note! We are experiencing issues with lab result viewing in the Patient Portal.  We are working with the vendor to address the issue.  In the meantime, if you are unable to view your lab results within 10 minutes after clicking “My Chart”, Request Health Record please send us a secure message in portal and we will be happy to send a copy of the results to your portal inbox. 

  • To send a secure message log in to the patient portal, hover over the mail tab and click compose message.
  • Select, “I have a medical question”.
  • Complete remaining fields as requested and click submit to send your message.

On the portal, you'll be able to:

  • schedule an appointment

  • view your statement

  • pay your bill

  • ask a medical question

  • and view your lab results!

Not a member, or no enrollment token?

You can enroll in the patient portal at your next appointment, or you can request a token enrollment by calling the Portal Enrollment Team at 844-806-8391.
