PPNC is proud to provide you with your health care needs. 

We welcome all patients, regardless of age, income level, race, ethnicity, immigration status, gender and sexual orientation. Many of our staff speak Spanish, and translation services are available for other languages.

PPNC is here to provide you compassionate, high-quality, and comprehensive care — whether you’re insured or not. 

We accept many insurance plans, including Medicaid, but for those without insurance, we have a sliding fee scale as well as staff that can help you enroll in an insurance program. Whatever service you’re here to receive, we’re glad you’re here and are happy to help you identify resources and services that fit your needs. To read more about fees & payment options, click here.


Our Team of Medical Staff

Our team of medical staff includes doctors, nurses and advanced practice clinicians who can coordinate your care across multiple health care settings, such as with behavioral health providers, specialists and hospitals. Based on your needs, our team will provide you with the highest quality, evidence-based care, educational and self-management support for a range of health care needs. On-site, our integrated behavioral health providers – a social worker or counselor – are here to provide services that focus on your mental and emotional well-being.  Along with our support staff, they are here to serve you.

As noted above, you can schedule future appointments by calling 516-750-2500 (Monday – Friday from 8:30am – 7pm, Saturday 8:30am to 2:30pm).  If you have a medical emergency when our office is closed, please call the same number (above), and the provider on-call will be able to address your health care needs. Our providers have 24/7 access to your medical record. 


Medical History

In order to meet our goal of providing optimal care, it is important that you provide a complete medical history and share information about care obtained outside of our practice.  If you see any doctor outside of Planned Parenthood, undergo surgery, or visit an emergency room, please call our Call Center at 516-750-2500. Our staff will work with you to ensure that all information is shared safely across all involved providers. 

If you have previous medical records from prior care received from other medical providers, please let us know so that we can obtain the most recent, up-to-date information about your health. If you are in the office for a visit, you can also ask our staff for the necessary forms to request chart information from your other medical providers.

  • No pets are allowed in our health center. Service animals specifically trained to aid a person with a disability are welcome.
  • Electronic devices, including cell phones, are not allowed during patient visits.
