Thank you for choosing PPHP for your health care needs. Before requesting your appointment, here are a few things to know: 

  1. You are not able to request an appointment online for an abortion, prenatal services, or colposcopy. To schedule those services, please call (800) 230-PLAN.
     
  2. If you have health insurance, please enter the Insurance Company, your Member Number, and Group Number in the Information for Provider Box.
     
  3. You are able to request an appointment for a day and time frame, not a specific time. A specific time will be emailed to you based on your request and the service you are requesting.
     
  4. You will need to provide a valid telephone number and verify this before booking an appointment. Any number is acceptable, though cell phone is recommended so we can ensure confidential communications and also text reminders and info to you if needed.
