North Carolina Requirements
There is no waiting period. Minors must come with a parent or legal guardian who will also consent to the procedure. This person must stay until ultrasound is done AND participate in the Informed Consent Session to sign procedure specific consent forms. We must have a photo ID WITH DATE OF BIRTH for the parent, a current photo ID WITH DATE OF BIRTH for the minor, and a Birth Certificate or other legal document that states that the person accompanying the minor is legally allowed to consent to the procedure. If the minor has a school ID without a date of birth, we request a school transcript that documents her birth date. If a minor is calling to schedule the appointment herself, we require her to have her parent contact us to be told this information. Photo ID of minor is subject to review. If a minor does not have any photo ID, then it must be obtained from the DMV.
Obtaining a North Carolina Identification Card: Any resident of North Carolina can be issued a special identification card provided he or she does not have a valid driver's license. The cost for an ID is $10
Minors may contact their Clerk of Court and obtain a judicial bypass if they are unable to obtain parental consent OR they MUST do this if they are unable to provide all of the necessary documentation at the time of the appointment.
If you are under the age of 18 and need help talking with your parents/caregivers or seeking judicial bypass we can help.
