The Patient Portal is a secure online website that gives you convenient  24-hour access to your personal health information from anywhere with an internet connection. Using a secure username and password, you can view health information such as recent medications, vital signs and lab results.

Getting Started

You will receive a printed page from the Health Center with instructions on how to access your patient portal and an enrollment code, called a token. This should be given to you during check-in at the Health Center or it may be with the materials you receive when you check out. You will also received an email from [email protected] with these same instructions.

Requesting Access to Medical Records and Lab Results

Once you have set up your account and logged into you patient portal, follow these instructions below to request that our automated system send your record to you Patient Portal. In some cases, this process may take up to 24 hours.

From a laptop or computer:

  • In the top menu, hover over the “My Chart” option.

  • From the drop-down, select “Request Health Record.”

  • On the following screen, verify that the practice and date ranges match what you want and click “Submit”.

  • You will receive an email when the records have been uploaded to your Patient Portal. You can view your records by hovering over the “My Chart” tab and clicking “View My Chart”.

From a mobile device:

  • In the upper left hand corner, tap the ☰ icon.
  • Select “Request Health Record” from the menu.
  • Select “OK” on the pop-up screen. You may use the checkbox to opt out of seeing this message in the future.
  • Verify that the drop-down information is correct and then select “Submit”.
  • You will receive an email when the records have been uploaded to your Patient Portal. You can access the records using the “Chart” button at the bottom of the screen. You may need to use the right and left arrows to find the button.

From a laptop or computer:

  • In the top menu, hover over the “Mail” option.

  • From the drop-down, select “Compose Message.”

From a mobile device:

  • Tap the icon in the upper right hand corner to compose a message.
