Preparing for Your Visit

To ensure the best experience possible we have provided a list of instructions to help you prepare for your visit. Instructions for Your Visit

  1. We encourage patients to pre-register for their upcoming appointment on MyChart, our patient portal. MyChart allows you to upload your insurance information for verification prior to your visit and will ensure that your provider accepts your insurance. 
  2. If you are not registered for MYCHART, we recommend that you call 855-207-7526 prior to your appointment to verify your insurance coverage for your appointment and to expedite your check-in process upon arrival to the health center
  3. We ask that you do not bring children with you to your appointment - for their safety, children cannot go into the back office. 
  4. A support person is welcome to accompany a patient through certain portions of the patient visit at the discretion of the Health Center Manager and if clinic flow/capacity allows. Support persons must bring a photo ID and will be asked to exit the clinic for any inappropriate or aggressive behavior.

  5. Cell phones may not be used during certain clinical care portions of visit.
  6. Masks are not required for staff and patients in PPAZ health centers when Community Transmission levels of COVID are low. Patients have the right to ask medical staff to wear a mask.
  7. Please note that if your scheduled provider is ill or not able to come in on the day of your appointment, another provider will be assigned for that day, with little notice. You may call the health center on the day of your appointment to confirm your insurance details and provider availability. 

Insurance Plans

If you plan on using insurance, please have your insurance information available when booking your appointment. To expedite your check-in experience, please ensure you’ve provided us with your insurance information before your visit. You must bring in your insurance card and photo ID in order for us to bill your insurance. Co-pays and deductible amounts are due at time of service. Planned Parenthood Arizona accepts the following health insurance plans:

  • Arizona Complete Health AHCCCS

  • United Healthcare Community AHCCCS 

If you do not have insurance, you may be able to receive coverage in the health care marketplace under the Affordable Care Act.

Your Payment Options

Self-Pay

Payment is expected at the time of service (cash, credit card, or money order; no personal checks, please). If you are using a credit card, you will need to provide photo ID.  If you plan to pay for your visit with a card that is not your own, the card owner must be there with you and must also be prepared to present their photo ID.
