How do I make an appointment?
To schedule an appointment, call 1-800-230-PLAN or find a health center to schedule online. Both in-person and telehealth visits are available.
How much does it cost?
Regardless of your insurance status, our staff is ready to help you figure out costs and payments for primary care services.

How old do I have to be to access Primary Care at PPNCNY?
Patients must be 18 and over to access primary care services. If you are under 18, we can refer you to other providers who can administer care.
