If you are a patient of Planned Parenthood of the North Country New York, Inc. and would like to request a copy of your health records or have us send your information to another provider, you may request your patient health information (PHI) in the following ways:

  • Via your patient portal 
  • In person
  • Via mail or email
  • Via phone by calling a representative at 1-800-230-PLAN
  • Requesting an electronic copy
  • By filling out the authorization form for release of health information and returning it to your local health center (choose from the list below). We will respond to your request as soon as reasonably possible.

* If you are enrolled in our Patient Portal, you can access certain categories of your protected health information there. If you aren't currently enrolled in our Patient Portal but would like to be, contact your local health center.
