You Have the Right to a Cost Estimate

If you do not have health insurance or you do not wish to use your health insurance, you are entitled to know what PPMW will charge you for the care you seek.  PPMW will send you (by email or mail) a good faith estimate of charges after you schedule your appointment.  Or, if you are not ready to schedule an appointment, we will provide a good faith estimate of charges if you call to request one.  The estimate will include the care you ask to receive along with care related to those services.  If you want additional care during the appointment, the total costs will likely be more than the estimate we send you.  PPMW discounts still apply based on need and age.  


If your scheduled appointment is less than 3 days away, PPMW will provide a verbal estimate of charges, but you will not receive a written estimate prior to the appointment.
