Insurance information

Some services may be covered by your insurance. If you want to use your insurance, please be sure to have your insurance information at the time of your visit.

Medicaid, Medicaid Family Planning Benefits Program, and Medicaid for Pregnant Women accepted.

If you are uninsured, you may be eligible to receive financial assistance for your procedure.

If you are eligible for financial assistance you may be asked to bring documentation with you to the health center.

Important: You must be located in Ohio to receive virtual care or medications.

Appointments

You can privately and easily book your appointment online for select services 24/7.

To access your virtual telehealth visit in Ohio:

  • Prior to your appointment, please create your MyChart account using the link that was emailed to you. 
  • At least fifteen minutes before your appointment, log into your MyChart account, and complete your eCheck-In using the green button next to your appointment information. 
  • At your appointment time, click the green "Begin Visit" button.
    • You can connect with a computer, tablet, or smartphone. You must allow access your camera and microphone.
  • Once your provider joins the waiting room, you will be connected, and the appointment will begin.

Important: You are scheduling a telehealth appointment which will not take place at the health center.
