Only established Gender-Affirming Hormone Therapy patients are able to schedule appointments online at this time. If you are a new patient, please call 800.230.PLAN.

Some services may be covered by your insurance. If you want to use your insurance, please be sure to have your insurance information at the time of your visit.
If you are uninsured, you may be eligible to receive financial assistance for your procedure.
If you are eligible for financial assistance you may be asked to bring documentation with you to the health center.
- A Telehealth appointment costs $62-80
- Feminizing hormones cost $199 for established patients; $252 for new patients.
- Masculinizing hormones cost $226 for established patients; $221 for new patients.
