The Patient Portal is an easy and convenient way to manage your health information and communicate privately with your care team. The Patient Portal is available 24 hours a day, 7 days a week, so you are able to access your information, ask questions, and make requests on your schedule!

View care summaries, test results, medication lists, and vaccination history
Privately and securely send messages to your provider and care team
Request prescription and medication refills
Make an appointment or view your appointment history
Pay bills and manage your statements
Please Note: Due to a new Federal Act to increase access to health information, some lab results are released to the portal as soon as they are completed by the lab.
Please allow up to three business days for us to contact you via portal, phone or letter if the result is abnormal and requires follow up. Please be assured that any results that require immediate action will be brought to the clinician’s attention in a timely manner. If you’d like to speak to someone about a lab result, you can call at 1-800-230-PLAN (7526).
