Community Health Services

Planned Parenthood Northern California has one of the largest education and community services programs in the country. We provide comprehensive, responsible, age appropriate sexuality education, behavioral health, and patient navigation support.

We recognize that social determinants of health profoundly affect our patients’ well-being and health outcomes. The goal of our community health initiatives is to address health disparities and bridge gaps in accessing care and resources to promote healthier and more equitable communities.

Our Programs and Services

Community Health Worker Program

Community Health Workers (CHW) work to improve the health and wellbeing of communities through patient navigation, health education, and outreach at local events.

Education and Training
 

We provide sexual health education to youth and adults; trainings for parents, teachers, and providers; peer education programming; and community outreach services.

Behavioral Health

We provide quality mental health care to the communities our health centers serve. Our counselors are able to meet with patients during their medical visits, provide in-the-moment counseling for immediate needs, and offer ongoing, individual counseling in-person or via teletherapy.
