We value our patients and their time. Whether you’re insured or not, patients can make an appointment and access a number of primary care services, including:

  • Sinus infection
  • Ear infection
  • Pharyngitis (sore throat) 
  • Contact dermatitis (rashes)
  • Acute gastroenteritis (upset stomach)
  • Acne
  • Irritable Bowel Syndrome (IBS)
  • Seasonal allergies

How do I make an appointment?

To schedule an appointment, call 1-800-230-PLAN or schedule your visit online. Currently, only in-person visits are available for primary care.

Regardless of your insurance status, our staff is ready to help you navigate costs and payments for primary care services: please call 1-800-230-PLAN.

How old do I have to be to access primary care at PPGP?

Currently, patients must be 18 and over to access primary care services. If you are under 18, we can refer you to other providers who can administer care.
