When you check in for your visit, bring:
- A completed the funding application. If you can't complete the application before your visit, we'll help you fill it out at check-in.
- A photo ID.
- A month's worth of paystubs or other proof of income, such as:
- Current W2 or tax return
- Copies of business records
- Financial award letter
- Court order
- Written statement from any person or company that sends you income

To apply to keep your coverage all year and continue getting birth control at no-cost:
Be sure to complete the full application and bring your photo ID and proof of income to your appointment. If you can’t bring the needed documents, we’ll help you with follow-up steps.
If your short-term coverage ends:
You can only receive short-term coverage once per year. If your short-term coverage ends, you can still apply to receive a full year of coverage by submitting a complete application and documentation to:
