On November 1, 2023, PPSWCF changed its electronic medical record systems to Epic. You can now access your records from your Epic MyChart account.

TO ACCESS YOUR PPSWCF MYCHART ACCOUNT:

Check your email inbox for an email inviting you to join MyChart. If you're having trouble finding it, try searching "[email protected]." If you do not have the email invitation, please call 941-365-3913 to request your link to join. 

Once you have set up your MyChart account, you can return to this page and use the button below to log into your account or access it through the MyChart app. 

In order to access your records prior to November 1, 2023, you must send a request through the MyChart Message Center. Simply start a new message, select Customer Service Question, and submit a Release of Information Request. You can also request your records by calling us at 941-567-3800. to speak with a PPCWSF Medical Records representative.
