About Your Visit

Required for your Appointment

  • You must be in the state of Michigan during your appointment.
  • Register for MyChart. A registration link will be sent to your email. We recommend that you download the MyChart app if you are using a mobile device.
  • Allow at least 15 minutes before your appointment start time to complete the check-in process.
  • For birth control prescriptions, please have a blood pressure result from within the last three months.
  • For gender-affirming hormone therapy appointments, you must watch the video related to your care prior to your appointment. If you do not watch the video before your visit, your visit will be paused for you to watch, which will extend your time with us.

Fees, Payment, and Insurance

  • If using insurance, please bring a photo ID and your insurance card to your appointment.
  • Many insurance plans, including Michigan Medicaid, are accepted. We also offer an income-based sliding fee scale for patients. All fees, including copays, are due at the time of service.
  • If you do not have insurance, please visit MI Bridges to determine if you are eligible for insurance through the State of Michigan.
  • If you have questions regarding what services are covered by your insurance, please contact your insurance company. We will bill your insurance for the services you receive where applicable.

If Your Visit Is for Medication Abortion

It is illegal under Michigan law to coerce a person to have an abortion. You will be asked questions about coercion at the time of your appointment. 

Required for Medication Abortion

  • State of Michigan Informed Consent for Abortion paperwork is required by law. We cannot see you for your appointment without this paperwork. There are two ways to access this information. Once you access the form, you must upload it to your MyChart account.
    • Pick up the paperwork at any Planned Parenthood health center in Michigan. Upload a picture of the first page of the packet; the time and date must be visible on the form when uploaded. Alternatively, you may ask the health center staff to scan it into your MyChart account.
    • Access the paperwork online. Click the button that says "Finish," then print the confirmation form as a PDF. If you can, please fill out and sign before uploading to your MyChart account.
  • You must bring your picture ID including date of birth.
  • Prior to the abortion you must watch three short videos. You will have access to the videos in your MyChart account by clicking on your Appointment Instructions. If you do not watch the videos before your visit, your visit will be paused for you to watch, which will extend your time with us.

Fees, Payment, and Insurance

  • The cost for a medication abortion is $600.
  • If you need financial assistance, please message us in MyChart or call us at 800-230-PLAN to discuss your options.
  • If you are using insurance, we need the insurance information prior to the appointment. Please call us if you did not provide that information when scheduling your appointment. Bring a photo ID and your insurance card to your appointment.
  • If you have questions regarding what services are covered by your insurance, please contact your insurance company. We will bill your insurance for the services you receive where applicable.
