We strongly recommend that you enroll in our MyChart patient portal prior to your visit. The portal will allow for you to complete your registration information and medical history prior to your visit in a private space. It will also allow you to use the eCheck-in feature which could shorten the time of your visit. If you did not request MyChart enrollment when making your appointment, or if you have questions about MyChart, please call us at 1-800-230-PLAN.
We ask that you do not bring children to your in-person visit. We know that this is sometimes unavoidable, and we will do our best to provide you the care you need while ensuring the safety of you and everyone in our health centers.
You will be screened for COVID symptoms and exposure when you arrive. If you have any symptoms of COVID-19 or a recent high-risk exposure, please cancel and reschedule your appointment when resolved.
