If you have been a patient at any Planned Parenthood Keystone health center and would like a copy of your records for yourself or your health care provider, you can download and fill out this Medical Records Release Form, then fax it, mail it, or bring it to your closest health center. If you are uninsured or do not want to use your insurance, you have the right to receive a "Good Faith Estimate" explaining how much your medical care will cost.

Under the No Surprises Act, healthcare providers need to give patients who don't have insurance or who are not using insurance, an estimate of the bill for medical items and services.
