Medical Record Request 

Patients may request a copy of their medical record or ask to send it to someone else.

  • To safeguard your privacy, complete and sign a Medical Record Release form.

The form may be submitted via fax to (267) 668-2630 or mailed to Medical Records at 1144 Locust Street, Philadelphia, PA 19107.

You can also access your medical records via MyChart.

  • For any records unavailable on MyChart you can request them within the customer service section of MyChart, under release of information.
  • Write a brief description as to what records you want released and where you would like them sent.

*PPSP may charge fees allowable under State law and the Federal Health Insurance Portability and Accountability Act (HIPAA). We do not charge a fee to release medical records to physicians or other health care facilities. There may be a fee to release copies for personal use or to release copies to a third party.
