How to Make Your Appointment Online

Using your temporary login and password (existing patients only)

If you already have a permanent portal login, please start here instead. 

Download detailed instructions or follow the steps below: 


Step 1 – Review the invitation email that you received with a temporary username and password.

Step 2 – Follow the directions in the invitation email:  

  1. Click the link in the email or go to login.my-appointment.org
  2. Click the button “I AM NEW HERE”
     

Step 3 – Read the Terms and Conditions and accept them.
 

Step 4 – Login using your temporary username and password:

  • Enter your temporary username (all numbers)
  • Enter your temporary password, following the detailed instructions in the email for how to build your temporary password:
  • For example, if your name is John Doe and your birthday is January 5, 1975, then your temporary password is: JD01051975.

 
Step 5 – Enter the favorite color that was included in the introduction email.
 

Step 6 – Create a Username and Password


Step 7 – Login to the Patient Portal
 

Step 8 – Select “Schedule an Appointment” near the center of the screen.

  • Login on the left-hand side of the screen under “Already a Member?”
     

Step 9 - Enter information under “Select Provider and Location.” Then click “SEARCH.”
 

Step 10 – Depending on your criteria, a list of appointments will display:

  • Choose the one that fits your schedule best
  • Click “BOOK APPOINTMENT”
     

Step 11 - A confirmation screen will appear. You can print it for your records.
