What do I bring to my appointment? 

  1. Photo I.D.
  2. If you have insurance, please bring your insurance card.
  3. If you do not have insurance and you are age 20 or older, please bring two pieces of proof of income. Proof of income includes:
    • Pay stub
    • Social security check
    • Disability check
    • Workmen’s compensation
    • Unemployment check
    • Grants
    • Alimony/child support
    • Bank statements
    • W-2
  4. Medical records from previous health clinics or doctors’ office, if any. (This is especially helpful if you wish to continue or get started on a birth control method, including the IUD).
  5. Any reproductive or sexual health questions you have.
