Kansas Consent Forms
Comprehensive Health of Planned Parenthood Great Plains requires unemancipated minors, under the age of 18, seeking an abortion to obtain notarized written consent of the minor and both parents (if the parents are married) or legal guardian(s) of the minor.
Patients no longer need to bring a 24-hour consent form to their appointment. A recent Kansas court ruling struck down several medically unnecessary restrictions including the state-mandated form. Minors from Missouri must also complete an additional section at the end of the Notarized Minor and Parental Consent form, which is labeled "Notarized Attestation of Second Parent Notice," before their appointment.
