Appointments
You can now self schedule your appointments. This scheduling service is available 24/7 to fit your timing and lifestyle. Click here to get started. If you do not see your preferred appointment type, please call us at (801) 532-1586 to schedule a visit.
We have an increase in people handing out disinformation at our entrances. We ask that you do not stop, talk to, or take handouts from anyone outside when pulling into the driveway or parking lot or walking up to the building. Please come into the health center, and our staff will be happy to answer any questions you have.
