Please check-in 15 minutes prior to your scheduled appointment.
Childcare Policies
Only infants and toddlers in a car seat or stroller are allowed in the exam room. Please make arrangements for childcare for any other children.

You can fill your birth control prescriptions at our health center instead of the pharmacy. During office hours

Symptoms of COVID-19 (coronavirus) include fever, cough, sore throat, and difficulty breathing. ANYONE WITH THESE SYMPTOMS WILL NOT BE ALLOWED TO ENTER THE HEALTH CENTER. If you have any of these symptoms or have been exposed to a person with a confirmed case, please do not come in to the health center. Call 800-230-7526 to reschedule your appointment. Contact your primary care provider for further assistance.
All staff, visitors, and patients in PPMI health centers and buildings are required to wear a surgical or KN95/N95 mask while in the building. Cloth masks do not meet our safety requirement. Surgical masks are available at the front desk.
