Additional Information

More Appointment Information

We’re unable to provide childcare at our health center. For safety reasons children need to be accompanied by an adult when they’re in the waiting room. Only infants can accompany you to the exam room. Please bring a friend or family member to be with your children if you don’t have childcare at home.

You can fill your birth control prescriptions at our health center instead of the pharmacy. Supply visits are available during all business hours.

Only infants and toddlers in a car seat or stroller are allowed in the exam room. Please make arrangements for childcare for any other children.

You can come to us for services that don’t require a medical exam. For example, birth control education and supplies; emergency contraception (AKA the morning after pill); pregnancy testing and counseling; STD testing, and testing and treatments for urinary tract infections.

Other Health Center Information

We look forward to seeing you at your appointment! Before your appointment please visit our eForms and complete our secure health history electronic form.

No registration or account creation is required. Completing the form will give our staff valuable information about your medical, family, and social history and will also save you time during your health center visit.

Please make sure you have a list of your current medications when you complete the form. At the end of the form be sure to submit the information. Submitted information will be able to be securely accessed by our staff once you arrive at the health center for your appointment.
