We accept many private and public insurance plans. If you don't see your insurance listed, please call us at 239-481-9999.

Medicaid Options

Medicaid, Medicaid Family Planning Benefits Program, and Medicaid for Pregnant Women accepted.

That includes Medicaid managed care plans, however there may be limitations by county. The plans accepted at this Health Center Location are listed below: Humana Medical Comp Plan Molina Vivida Health Staywell/Wellcare/Healthease Sunshine.

Services Coverage

Most health insurance plans now cover prescription birth control, annual wellness exams, and STI/HIV screenings with no copay, and many other services with some copay required. Contact your health insurance company directly to make sure that the services you need are covered and what, if any, out of pocket costs you might have.

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost 
 
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
 

  •  You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees. 
  • If you schedule your service at least three days in advance, make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service. 
  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. 
  • Make sure to save a copy or picture of your Good Faith Estimate.

Appointment Policies and Information

We now offer many services through Telehealth. You can schedule a virtual visit for birth control, STI Testing, PrEP, Gender Affirming Hormone Therapy, infection checks, and pregnancy tests.


All Patients must bring an acceptable form of ID with you at the time of your appointment. Whichever form of ID you choose to bring must be an original copy and cannot be damaged or altered in any way. Acceptable forms of government issued photo ID are listed below.

• Driver’s License
• State issued ID card
• Passport
• Military ID card

If you do not have a government issued photo ID, you must bring one form of photo ID (like a school ID card) and a form of identification that proves your age (Like a birth certificate).

Childcare Policies

We’re unable to provide childcare at our health center. For safety reasons children need to be accompanied by an adult when they’re in the waiting room. Only infants can accompany you to the exam room. Please bring a friend or family member to be with your children if you don’t have childcare at home.

Only infants and toddlers in a car seat or stroller are allowed in the exam room. Please make arrangements for childcare for any other children.

Services Offered

You can fill your birth control prescriptions at our health center instead of the pharmacy. Patients can call ahead to request a refill two hours or more before they wish to pick it up to make sure it's ready when they arrive. Mail-A-Pill is available to patients with a current prescription for some of the birth control pills we offer in the Health Center. Mail-A-Pill is the easy and convenient way to have your birth control shipped directly to your door. Call us for more details on how you can sign up.
