The Family Planning Benefit Program

Can't juggle it all? We can help! FREE services are available to many women and men who do not have health insurance—or who have only partial insurance coverage—through the Family Planning Benefit Program.

This free insurance program covers GYN exams, Pap smear screening for cervical cancer, birth control information and supplies, and even testing and treatment for sexually transmitted infections (STIs, also known as STDs) if this is done as a part of a birth control/family planning visit. If you're eligible for coverage, all of these services are completely free with no co-pays or out-of-pocket. If your enrollment counselor determines that you are not eligible for coverage, you can still receive services at a reduced cost based on your level of income.

Signing up for free insurance coverage is easier than ever! Just come fifteen minutes early to your health center appointment with the required documents, or set up an appointment to enroll. Click here to request an appointment online, or call (518) 434-5678 to set up an appointment right away and get more information on covered services, necessary documents and eligibility.

And now you can come prepared! Click here to download a copy of the Family Planning Benefit Program application, and here for application instructions. If you need help, an enrollment counselor will be happy to assist you at the health center. La aplicación en español está aquí, e instrucciones para la aplicación están aquí.
