Abortion Access for Minors - Parental Notification Forms



If you are pregnant and age 17 and under in order for you to have a legal abortion, a new state law must be satisfied that one of your parents or legal guardian is notified that you are planning on having an abortion.

The Parental Notice of Abortion Act, which became effective on June 30, 2006 requires all minor femails, age 17 and under, to undergo a process for securing a safe and legal abortion.  Please call your local clinic so they can advise you on your next steps and which form listed below you will need to download and fill out to begin the process.
