Planned Parenthood of South Central Michigan is the only health care provider in the state of Michigan that has financial assistance available for clients to help pay for sterilization surgeries. Financial assistance is determined by household income and the number of dependents supported by that income. The assistance program uses a sliding scale to determine how much each applicant will pay according to State of Michigan guidelines.
The State mandates that each program applicant complies with the following guidelines:
- Each applicant must be 21 years or older;
- Each applicant must go through an informed consultation 31 days prior to their surgery.
If you are using the assistance program and do not live in the Kalamazoo area, after we receive your application we will send you a letter telling you how much your surgery will be and include the name, place, and number to call to schedule your informed consultation so you will only have to travel to Kalamazoo for the day of your surgery.
Please note that the application asks for weekly household income. If it is easier to calculate your bi-weekly or monthly income, then please note that after the amount. If there is no indication, the amount put on that line will be assumed as the weekly household income.
For women interested in a tubal ligation, in addition to the informed consultation, we also need results from a current Pap exam, as well, as results from a gonorrhea and chlamydia test, within 6 months.
If you have Medicaid and are interested in a tubal ligation, please contact your caseworker before applying for the assistance. We do not accept Medicaid clients for this surgery at this office, because the State will not allow two state funded programs to pay for one visit. Your caseworker will know who you need to contact for the procedure. If your Medicaid plan does not cover tubal ligation, you may use the assistance program.
