In Mahoning County alone, African American women are twice as likely to have a baby that is considered very low birth weight than the population in general. Even more disturbing, the infant mortality rate in the African American community is twice the rate of that in the population in general.

Our Healthy Moms, Healthy Babies program can help. Healthy Moms, Healthy Babies is a neighborhood outreach program that targets pregnant, high-risk African American women in Mahoning County. Community Care Coordinators provide in-home visits during their client's pregnancy and until their baby turns two years old, and work to empower communities to help eliminate health disparities. The program is specifically tailored to the needs of each client and serves as a vital link to other local human service agencies. The focus of the program is to encourage early and consistent prenatal care.

Our Healthy Moms, Healthy Babies program is one of 12 Infant Mortality Reduction Initiatives in Ohio whose mission is to prevent low and very low birth weight babies and infant mortality.

The Healthy Moms, Healthy Babies program addresses the barriers that at-risk pregnant women and their babies experience. Many of the women in our program don't have transportation to get to their prenatal appointments, they may not have any medical insurance, or they engage in risky behavior. Our staff helps their clients recognize these potential problems and work with them to prevent poor health outcomes.
