Planned Parenthood Mohawk Hudson’s Ticonderoga health center at 171 Lake George Avenue will no longer be open after December 11. The center will only be open in the interim from 10 a.m. to 3 p.m. on November 20, December 4 and December 11. If you are seeking care on a day we are closed in Ticonderoga, we want to help. Planned Parenthood North Country New York will open a health center in Essex County early in 2014.
What about my prescriptions?
• If you continue your care with PPMH your prescription refills are still available at a drug store of your choice, at a PPMH Center or by mail from PPMH. You can request refills by using the Patient Portal, directly through your pharmacy, or by calling 518.792.0994 x202.
• If you choose to transfer to a different provider, PPMH can approve prescription renewals to cover your needs until you become established as a patient with your new provider. All renewals are subject to the medical judgment of the prescribing clinician.
If you choose to transfer to a different health provider:
PPMH has a list of area providers who are accepting new patients. There is a medical records release form you will need to sign for your new health care provider to receive your records. All patient health records are kept in accordance with Federal guidelines. By law, we can release your patient information only with your written authorization.
