Medical Coupon Customers
Medicaid is switching from paper “coupons” to a new plastic ID card
If you currently use a paper Medical Assistance ID (MAID) card that is sometimes referred to as the medical coupon you will need to look for your new free plastic ProviderOne “Services Card” in the mail. It will be delivered to the same address where you’ve previously received your paper medical coupons.
Your new ProviderOne “Services Card” will be the only Medicaid voucher accepted beginning in December of 2009.
Washington State’s Department of Social & Health Services reminds you that the card is changing, but the rules for eligibility and services are not.
