Use our online appointment system to request an appointment at one of our Western Washington health centers or call us directly at 1.800.230.PLAN. This is a service for non-urgent appointments.

My Account

Access your Planned Parenthood account here.  Create a new confidential account or log-in to your existing confidential account to review “My Messages”.  You can also change any of your existing account information including your password, user address, or preferred location.

Order Birth Control Online

Order your prescription Birth Control here.  You will need to:
1.) Create a new account or access your current Planned Parenthood account.
2.) Complete a health history form to purchase the birth control method of your choice - approximately 10 minutes.
3.) Wait up to 1 business day to receive a phone call from a Planned Parenthood nurse to review your health history.
4.) Indicate if want your birth control delivered to your house, phoned into a pharmacy, or if you prefer to pick up your birth control at your nearest Planned Parenthood health center.

Order Emergency Contraception (EC)

Order your Emergency Contraception (EC), the morning after pill, here.  You will need to:
1.) Create a new account or access your current Planned Parenthood account.
2.) Complete a health history form to purchase Emergency Contraception (EC) – approximately 10 minutes.
3.) Indicate that you want Emergency Contraception delivered to your house, phoned into your pharmacy, or if you prefer to pick up your birth control at your nearest Planned Parenthood health center.

Refill Birth Control Online

If you previously received birth control from one of our health centers or from the Online Health Center, you may request a refill here. Your prescription must be active - most prescriptions are written for 1 full year. There is a $5.00 charge for any refill if your prescription is not on file with the online health center.

Western Washington Health Forms

Appointment wait times may vary depending on the number of walk-in patients present and the types of service required.  To ensure the shortest delay, please download the appropriate forms and complete them prior to your visit.

All new/first time patients are required to fill out the following two forms:
• Notice of Health Information Privacy – HIPAA
• Notice of Health Information Privacy – HIPAA (Spanish)
• Request for Medical Services
• Request for Medical Services (Spanish)

All new/first time patients scheduled for an examination are required to complete a Self-Medical History form.  Existing patients should also complete this form if it’s been over a year since your last visit.

Miscellaneous Form:
Release of Health Information

For those patients who qualify, Take Charge applications must be completed in the Health Center at the time of visit.  You may qualify if you are a Washington resident, U.S. citizen and present and verify:
• Your birth certificate or passport;
• Your Social Security card or green card (& 5 years residency);
• Your income level; and
• You have no other Medicaid coverage.
