First Time Patients

If you have never been to a Planned Parenthood Golden Gate health center or if it has been more than two years since your last visit, please bring these forms:

Forms Needed to Request Your Medical Records

If you want your records released to a third party, fill out the Authorization for Release of Medical Records form to release your health information.

If you want your records released to yourself, fill out the Request for Access to Health Information form.
