#  Your body
#  Money (Cash, Visa, MasterCard, or check)
#  Photo I.D.
#  Insurance card (if you have health insurance)
#  Proof of citizenship (birth certificate or passport)*
#  A friend (optional, of course!) 

*All patients wanting to use funding programs must show proof of citizenship.  Please bring in your passport or birth certificate to ensure coverage of your visit.

Payment Options

• Medicaid and most private health insurance is accepted.
• If you are uninsured, you may qualify for a state-funded program or a lower fee scale. Please bring the following documents:
  • birth certificate
  • photo ID 

• Fees for services are based on your household income.

Payment is expected at time of service. We accept:

• cash
• major credit/debit cards
• money orders
