You have the right to access certain information concerning this facility by making an anonymous toll-free call to (888) 973-0022.

The toll-free telephone line can provide you with the following information:

• whether this abortion facility is licensed by TDSHS;
  
• the date of the last inspection of this facility by TDSHS, and any violations of law or rules discovered during that inspection that may pose a health risk to you;
  
• any relevant fine, penalty, or judgment rendered against this facility or a doctor who provides services at this facility.

TDSHS is the responsible agency for facility complaint investigations. Complaints about this facility may be registered with:

Complaints must be registered with the department in writing. A complainant must provide his/her name. All complaints will be kept confidential.

This facility is required to provide a woman seeking an abortion with a copy of the booklet "A Woman's Right to Know" and a resource directory, if she should choose to view them. These publications may also be accessed online at this link.
