Please note: if you would like to schedule a pap smear or well woman visit, we need you to let us know if you're experiencing any other gynecological issues, or if you've been exposed to an STI when you are scheduling your visit. A Pap smear will not return accurate results until those other issues have been treated.
SAVE TIME! Take 10 minutes before your health center visit, print and fill out the correct medical forms, then give it to our health center staff when you come to your first visit. Please DO NOT SIGN THE FORM until you are in the health center and our staff can witness your signature.
