MyChart  

Accessing your medical information and communicating with your PPSNE provider is made seamless through MyChart—an online platform dedicated to your healthcare needs.  

What can MyChart do for you?

Review summaries of your previous appointments, including issues addressed during each visit.

Connect with your care team when you need us, or meet with your provider for telehealth appointments.

View your medications, including dosage information and instructions, and request refills.

Pre-register with 'e-check in' and save time at your visit by submitting any necessary information in advance.

View new labs or test results.

Please Note: Due to a new Federal Act to increase access to health information, some lab results are released to the portal as soon as they are completed by the lab.

Please allow up to 3 business days for us to contact you via MyChart, phone or letter if the result is abnormal and requires follow up. Medically urgent results are addressed by our team as soon as possible. If you’d like to speak to someone about a lab result, you can call at 1-800-230-PLAN (7526)

 

How to Get Started

Whether you're a new or existing patient, we strongly encourage everyone enroll in MyChart.

1. Create Your Account: In order to create a MyChart account, you need to have an activation code. There are two ways to receive an activation code: 

  • If you have an upcoming visit, your care team can send you an email with an activation code after your appointment. 
  • If you do not have an upcoming visit, you can call our call center or health centers at 1-800-230-7526 to request an activation code. You must be a current patient and have an email address. 

2. Use Your Activation Code: Click "Sign Up Now" in the New User section to enter your activation code and other personal verification items, such as the last four digits of your Social Security number and your date of birth.  

3. Create Your MyChart Username: Your username should be something that others wouldn't be likely to guess but easy for you to remember. It cannot be changed at any time. 

4. Create Your Password: Your password should be a unique combination of numbers and letters, using both uppercase and lowercase letters. Your password must be at least eight characters and must be different from your MyChart username. Choose a password that you don't use for other websites. 

5. Set Your Security Question: This question will be used to verify your identity if you forget your MyChart password. Choose a security question from the list and enter your answer. Your answer cannot include your MyChart password. 

6. Enable Notifications: Finally, choose whether you want to receive a notification message in your personal email when there is new information available in your MyChart account. If you opt to receive email alerts, enter your email address.
