Free Birth Control & Annual Exams

mtbhometakecharge.jpgTake Charge is a program that provides no-cost reproductive health and family planning services to teens, men and women in order to prevent unintended pregnancies.

Take Charge program is administered by the Washington State Health Care Authority. Planned Parenthood does not approve or deny your application. We fax completed applications to the Health Care Authority for approval.

To qualify, you must: Have income at or below 250% of the Federal Poverty level. For example: $2,329 for a single person and $4,803 a month for a family of four (see chart below).

  • Be a U.S. citizen or have a green card,
  • Be a resident of Washington State,
  • If you are aged 17 or under, you only need to report your own income,
  • Be in need of family planning services but have no family planning coverage through your health insurance plan or Medicaid (exception for clients up to 18 years of age and survivor of domestic violence who wish to maintain confidentiality regarding their use of family planning services.)

Speed up your visit by downloading the Take Charge enrollment form, complete and bring with you to your visit. If you prefer, MBPP staff will assist you with enrollment in Take Charge program at our health centers. Your coverage is for one year, with subsequent renewals for one year periods.  Along with your completed Take Charge application please bring the following with you to your appointment:

  • Proof of Income: Pay stubs from the previous 30 days. If you are 17 years of age or younger, you do not need to report parents' income. Patients receiving unemployment benefits or making less than $500 per month need to provide an explanation of how they are meeting their needs.
  • Proof of Citizenship/Residency: State or Government issued identification and a valid Washington state address. A valid social security number and an unexpired LPR or "green card" if you are a permanent resident.
  • Proof of Insurance: If you have other medical insurance, your private insurance will be billed first. If your insurance does not cover your family planning services, and you have been enrolled in Take Charge, we will bill Take Charge. If charges are denied by both payers, you will receive a bill from us. Your charges will be based on your family size and income level (see Sliding scale services).  

NO-COST family planning services include: 

  • Annual exam, including routine Gonorrhea and Chlamydia testing and treatment for women between the ages of 13 and 25, and pap smear,
  • Pregnancy testing,
  • All birth control methods approved by FDA, including birth control pills, Depo-Provera, IUDs including Mirena, The Patch, Nuvaring and emergency contraception ,
  • Non-prescription products like male and female condoms, contraceptive gels, foam, film, cream and suppositories,
  • Sterilization - vasectomy and tubal ligation.  

If you qualify, you pay nothing for birth control methods and family planning services covered under the Take Charge program. Each visit must be for family planning services.

 

Take Charge does not cover:

  • The cost of follow-up examination for abnormal Pap smears
  • Immunizations, HIV testing and other STI testing, except as stated above
  • The treatment of STIs, including medication, unless the treatment is directly related to the successful use of the chosen birth control method (for example, IUD insertion)
  • Pregnancy related services including abortion (contact local DSHS Community Service Office for enrollment for full-scope Medicaid, which covers the costs of maternity care or abortions).

If you qualify, you pay nothing for birth control methods and family planning services covered under the Take Charge program. Each visit must be for family planning services.

What if I am denied for Take Charge coverage?

Planned Parenthood does not approve or deny your application. The Washington State Health Care Authority processes all Take Charge applications. If your application is denied by Health Care Authority, you are responsible for all charges incurred during your visit. Charges will be based on your family size and income level (see Sliding Scale Services).
