How Much Will It Cost?

Our mission is to provide reproductive healthcare to anyone who needs it, regardless of ability to pay.  There is a cost for all our services however  you may be eligible for a discount.  We charge on a sliding fee scale based on household income and number of people in the household.

SelectPlan for Women is a state-funded program that provides free family planning services.  Please fill out this application and bring it with you on your first visit to the medical center.  Other documents needed to apply for free services are:

  • proof of residency - a driver’s license or utility bill in your name
  • proof of income - paystubs for the last 30 days of current income or an income tax return
  • proof of U.S. citizenship - a passport, birth certificate or satisfactory immigration documents

    Your First Visit

    Your first visit can be scary - and confusing.  Write down the questions you want to ask us before you come.  Read more about Planned Parenthood services hereThe more you know the better you will feel!

    Things to bring with you:

  • Photo ID is required for all payments made by credit/debit card or check.  ID is not required if you pay in cash.  If you do not have a driver's license, you can provide a passport, or school ID (with photo).  If you do not have identification, we can still see you if you pay in cash and do not apply for reduced fee services.

  • Insurance Card - Most insurances are accepted.  Bring your card with you to every visit.

  • Payment is expected at the time of your visit.  You may qualify for services at a reduced rate. Complete the Financial Assessment Form below and bring to your appointment or call for more information.

    Several forms need to be completed at the time of your first visit. Plan ahead and save time!

    MEDICAL RELEASE FORM - only needed if you are transferring records 
    CONTACT INFORMATION - will establish you as a patient for recordkeeping and billing
    MEDICAL HISTORY FORM - needed for all patients so we are informed about your health
    FINANCIAL ASSESSMENT FORM - used to determine how much our services will cost you

    Get Birth Control by Mail
    If you are already a patient, contraceptive supplies (pills and patches) can be ordered, paid for and delivered by mail! You may only need to see a clinician once per year and then use PILL-BY-MAIL.

    Walk-In or Call for an Appointment
    No appointment necessary for EMERGENCY CONTRACEPTION, PREGNANCY TESTS or REFILL of BIRTH CONTROL SUPPLIES.  Some services are available by appointment only.
