Free Birth Control & Annual Exams
Take Charge is a five year program begun in 2001 that provides free reproductive health and family planning services to teens, men and women in order to prevent unintended pregnancies.
Have income at or below 200% of the Federal Poverty level. For example: $19,600 for a single person and $40,000 a year for a family of four. (see chart below)

Each visit must be for family planning services.
