Online Health Center

You can use the Online Health Center to schedule appointments at any PPKY health center.  If you’ve been to the Online Health Center before, you’ll be asked to log in. If this is your first visit, you’ll need to take a few minutes to create an account. Please be assured that your privacy is very important to us and all information is kept strictly confidential.

Please note: This is a service for non-urgent requests. If you have an emergency, please contact our health center or an emergency room near you.

After making an appointment, you can save time by downloading and filling out your medical paperwork prior to your appointment. To print medical forms, click here.  Don't forget to bring them to your appointment!
