Appointment Information
We do not provide childcare services. We request that you please make childcare arrangements.
Please arrive 15 minutes prior to your scheduled appointment to check in and complete paperwork.

- In consideration of other patients and due to limited space in our waiting room, please limit your accompanying driver(s) to one person.
Payment Information
We accept the following forms of payment:

Payment is expected at time of service.
- If you want to use your insurance, please be sure to bring in your insurance information at the time of your visit. Please make sure to contact your insurance before your appointment day to check if an authorization is needed. If an authorization is needed, make sure to bring it in with you on the day of your appointment.
- If you plan to pay for your visit with a credit card that is not your own, the cardholder must accompany you for the visit and provide the credit card and a photo ID.

Some charges associated with your services may be covered by your insurance. If you want to use your insurance, please be sure to bring in your insurance information at the time of your visit.
