Health Center Services and Information

Walk-in Services

WALK IN SERVICES: Tuesday- Saturday for PREGNANCY TESTING, DEPO PROVERA SHOTS (PATIENTS ONLY), HIV TESTING (THURSDAY WALK IN)- APPOINTMENTS ARE ALWAYS RECOMMENDED FOR THESE VISITS ALL OTHER VISITS REQUIRE AN APPOINTMENT TEEN CLINIC WALK-IN - THURSDAY 2:30-5:30

All other services require an appointment.

If you need emergency contraception (morning after pill), you can walk in any time the health center is open.

Walk-in services may have to wait. Appointments are highly encouraged. Walk-in services are not available on Mondays. Please check the clinic hours and days for holiday or other closings.

Appointment Information

Teen clinic is available: Thursdays 2:30 - 5:30

Birth control refill pick ups are available ALL HOURS THE CLINIC IS OPEN~ DISCOUNTED BIRTH CONTROL IS ONLY AVAILABLE ON THURSDAY

At this health center you can come in for appointments that do not require a medical exam. For example, birth control education and supplies; emergency contraception (morning after pill); pregnancy testing and counseling; STD testing; testing and treatment for urinary tract infections

We do not provide childcare services. We request that you please make childcare arrangements.

Only infants and toddlers in a car seat or stroller are allowed into the exam room. We request that you please make arrangements for childcare for any other children.

Please arrive 15 minutes prior to your scheduled appointment to check in and complete paperwork.

Specialty Services

Pills by Mail is available at this health center — a service that delivers birth control pills to your home each month. Patients with a current prescription for some of the birth control pills that we offer at our health center can order Pills by Mail.

Other Health Center Information

Our health center has active protesters outside of the building. Please do not stop in the driveway or feel that you need to talk to the protesters. Protesters do not support Planned Parenthood or any of the services we provide. We also have volunteers, on many days, in yellow vests, that will be happy to escort you into the building. Thank you for your support.

Payment Information

Medicaid accepted.

If you are uninsured, you may qualify for a state-funded program or a lower fee scale. Please bring the following documents:

• pay stub
• proof of residence
• FOR SLIDING SCALE FAMILY PLANNING QUALIFICATION PATIENTS MUST PROVIDE ONE OF THE FOLLOWING: PAY STUB, W-2, AUTHORIZED LETTER FROM EMPLOYER, LETTER OF UNEMPLOYMENT-DISABILITY-CHILD SUPPORT-FOOD STAMP INCOME, OTHER DOCUMENTS AS APPROVED BY MANAGEMENT IF PATIENT CAN NOT PRODUCE THE ABOVE.

Fees for services are based on your household income.

Please bring proof of income when you arrive for your visit.

You will not be turned away because of your inability to pay.

We accept the following forms of payment:

• checks
• major credit/debit cards
• money orders

Payment is expected at time of service.

PAYMENT PLANS MUST BE MADE PRIOR TO COMING FOR A VISIT. PAYMENT PLANS ARE MADE FOR VISITS ONLY AND DO NOT INCLUDE MEDICATION, LAB FEES OR SPECIAL PROCEDURES. PAYMENT PLANS ARE GENERALLY ACCEPTED FOR FAMILY PLANNING OR WELL WOMAN EXAMS. HIV TESTING IS OFFERED FOR FREE- OTHER SEXUALLY TRANSMITTED TESTING MUST BE PAID IN FULL OR CAN QUALIFY FOR A SLIDING SCALE AT THE COLLIER COUNTY HEALTH DEPARTMENT 252-8200. PATIENTS ARE GENERALLY REQUIRED TO HAVE A VALID CREDIT CARD TO ARRANGE A PAYMENT AGREEMENT. NO PAYMENT PLANS ARE GUARANTEED.

EXPRESS STD CLINIC AND EXPRESS BIRTH CONTROL CLINIC OFFERED BY APPOINTMENT AND ON WEDNESDAY MORNING ONLY. BIRTH CONTROL EXPRESS $69.00 + BIRTH CONTROL = YOU SAVE A $30.00 STD EXPRESS CLINIC $99.00 INCLUDES GONORRHEA, CHLAMYDIA, SYPHILIS, & HIV TESTING = YOU SAVE $80.00

Insurance Information

The following insurance plans are accepted at this health center: FULL MEDICAID-FAMILY PLANNING MEDICAID

ALL BLUE CROSS/BLUE SHIELD INTEGRAL MEDICAID, MEDIPASS. SOME MEDICAID REQUIRES APPROVAL. PATIENTS ARE RESPONSIBLE FOR PAYMENTS THAT ARE NOT COVERED BY INSURANCE AND FOR CHECKING IF THEIR INSURANCE COVERS THEIR VISIT. SOME SERVICES REQUIRE AUTHORIZATION, NO GUARANTEE IS MADE FOR ACCEPTING INSURANCE. If you want to use your insurance, please be sure to bring in your insurance information at the time of your visit.
