From OH-82/Warren Outerbelt Road:
Take OH-5/Elm Road exit.
Heading North on OH-5/Elm Road exactly 1 mile.
Turn right into Elm Road Medical Park.
Proceed to rear of complex.
Enter Building C.

From Cortland, Ohio:
Head south on South High Street/Warren Road toward West Main Street.
Turn left onto South High Street/Mecca Street.
Take the 1st right onto OH-5 West.
Proceed 2.7 miles.
Turn left into Elm Road Medical Park.
Proceed to rear of complex.
Enter Building C.

Health Center Services and Information

Walk-in Services

Pregnancy tests are available on a walk-in basis.

All other services require an appointment.

If you need emergency contraception (morning after pill), you can walk in any time the health center is open.

Holidays

Our health center is closed for the following holidays:

  • New Year's Day
  • Martin Luther King Day
  • Memorial Day
  • Independence Day
  • Labor Day
  • Thanksgiving Day
  • Christmas Eve
  • Christmas Day

Appointment Information

Birth control refill pick ups are available any time the center is open.

At this health center you can come in for appointments that do not require a medical exam. These appointments include starting on birth control, STD testing, and testing and treatment for urinary tract infections. Please call to schedule an appointment.

We do not provide childcare services. We request that you please make childcare arrangements.

Children under the age of 10 are not allowed to wait unsupervised in the waiting area.

Only infants and toddlers in a car seat or stroller are allowed into the exam room. We request that you please make arrangements for childcare for any other children.

Please arrive 15 minutes prior to your scheduled appointment to check in and complete paperwork.

Specialty Services

Pills Now Pay Later® is available at this health center - a service that allows you to visit us for one appointment and walk out the door with up to a year's supply of birth control and a convenient, low, monthly payment plan.

Pills by Mail is available at this health center - a service that delivers birth control pills to your home each month. Patients with a current prescription for some of the birth control pills that we offer at our health center can order Pills by Mail.

Payment Information

Medicaid accepted.

Fees for services are based on your household income.

Please bring proof of income when you arrive for your visit.

You will not be turned away because of your inability to pay.

We accept the following forms of payment:

  • cash
  • checks
  • major credit/debit cards

Payment is expected at time of service.

Insurance Information

Please see the provided list of insurance plans to find out which ones include Planned Parenthood as an in-network provider.

Most health insurance plans now cover prescription birth control, annual wellness exams, and HIV and STI screenings with no copay, and many other services with some copay required. You should contact your health insurance company directly to confirm that the services you are interested in are covered, and what, if any, out-of-pocket costs you are required to pay.

Please be sure to bring your insurance card to your visit.
