What do I bring to the first appointment?
- If you do not have insurance and you are age 20 or older, please bring proof of income. Proof of income includes:
- Social security check
- Disability check
- Workmen’s compensation
- Unemployment check
- Grants
- Alimony/child support
- Bank statements
- W-2 - Medical records from previous health clinics or doctors’ office, if any. (This is especially helpful if you wish to continue or get started on a birth control method including IUD).
- Any reproductive or sexual health questions you have.
