What do I bring to the first appointment?

  1. If you have insurance, please bring your insurance card.

  2. If you do not have insurance and you are age 20 or older, please bring proof of income. Proof of income includes:
    Social security check
    Disability check
    Workmen’s compensation
    Unemployment check
    Grants
    Alimony/child support
    Bank statements
    W-2
  3. Medical records from previous health clinics or doctors’ office, if any. (This is especially helpful if you wish to continue or get started on a birth control method including IUD).

  4. Any reproductive or sexual health questions you have.
