DRIVING DIRECTIONS TO FREEHOLD HEALTH CENTER
From Route 9:
• Route 9 North or South to Route 537 West.
• ** Take 537W past Freehold Raceway Mall and Raintree Shopping Center on your right. Slow down when passing light at Iron Bridge Road and 537W (Diagonally across from CentraState Hospital; look for Wells Fargo Bank building (big red sign) on your left (800 W. Main) and make left turn into parking lot.
From Route 18:
• Route 18 South to Route 9 South. Proceed as above (**).
From Eastern Monmouth County:
• Take 537 West (Sycamore) which will become Main Street in Freehold.
• Take Main Street through town, past Raceway Mall and Raintree Shopping Center on your right. Slow down when passing light at Iron Bridge Road and 537W (Diagonally across from CentraState Hospital; look for Wells Fargo Bank building (big red sign) on your left (800 W. Main) and make left turn into parking lot.
From Route 33:
• Route 33 to Route 537 West.
• Proceed as above (**).
DRIVING DIRECTIONS TO HAZLET HEALTH CENTER
From Garden State Parkway:
• Take Garden State Parkway to Exit 117
• Bear right after exit on to Route 35 South.
• Continue to 3rd traffic light at Bethany Road. Turn right, Bethany Commons is on the left.
• Take 2nd driveway on left and park in area on right side of building.
• Suite 91 will be on your left.
DRIVING DIRECTIONS TO NEW BRUNSWICK HEALTH CENTER
From Route 18:
• **Take Route 18 North to Route 1 South past several shopping centers and Route 130 and get off at Jersey Avenue exit.
• Bear right and proceed down Jersey Avenue approximately 1 mile to traffic light for How Lane.
• Make a left on to How Lane.
• Go to second road on the right, which is Terminal Road. (First road is Home News).
• Make right onto Terminal and go to the end, which is Industrial Drive.
• Make a left turn and go to first parking lot on left. You will see a brown brick building, which is 10B Industrial Drive.
To Avoid Route 1:
• Take Garden State Parkway North or South to Route 287 North.
• Take Route 287 North to Exit 10, Easton Avenue.
• Follow signs towards New Brunswick.
• Continue on Easton to JFK Parkway.
• Turn right on to JFK Parkway – continue to Hamilton – turn left on Hamilton – continue 1 block to light.
• Turn right on Veronica to intersection of Route 27.
• Turn left, proceed ¼ mile to Industrial Drive.
• Turn right (Taco Bell on corner) – Planned Parenthood is 3rd diriveway past Taco Bell.
From Route 27/Highland Park
• Take Route 27 through downtown New Brunswick, (it becomes French Street).
• Continue down 27 approximately 4 miles. You will pass McDonalds, the Bus Station on the left, Tara Greens on right.
• Take left off 27 onto Industrial Way. Taco Bell is on the corner. Planned Parenthood is 2nd driveway on right.
From Garden State Parkway:
• Take Garden State Parkway to 440 South to 287 to Route 1 South – then follow directions in first paragraph (**) above.
From NJ Turnpike:
• NJ Turnpike – to Exit 9 (Route 18) – then follow directions in first paragraph (**) above.
From Trenton:
• Take Route 1 North to Livingston Avenue Exit – then follow directions in first paragraph (**) above.
DRIVING DIRECTIONS TO PERTH AMBOY HEALTH CENTER
From NJ Turnpike North:
• Take Exit 10. Go through toll booth and bear left towards signs Perth Amboy/Staten Island/440. Take 440 towards Perth Amboy (right lane); **take 1st exit for Smith Street.
• Follow signs to Smith Street. First traffic light after intersection of Smith Street and Convery Blvd (Route 35) is Herbert Street.
• Make a right on Herbert Street, which curves around and becomes Market Street.
• Approximately ¼ mile down on the left is 450 Market, which is on the corner of Market and Wilson. (Grey building on your left)
From 287 South:
• Follow signs “Business District”/Smith Street Exit.
• Proceed as above (**).
From Route 1:
• North to 514 East/Woodbridge/Raritan, past Middlesex County College and Raritan Civic Center to signs “Perth Amboy/440”.
• Take 1st exit for Smith Street.
• Proceed as above (**).
From Garden State Parkway North:
• Take Garden State Parkway North to Exit 125.
• Take Route 35 North.
• Go over bridge; make a right at first traffic light.
• This is Smith Street.
• Make a right on Herbert Street, which curves around and becomes Market Street.
• Approximately ¼ mile down on the left is 450 Market, which is on the corner of Market and Wilson. (Grey building on your left)
From Garden State Parkway South:
• Take Garden State Parkway South to Exit 129 for Route 9 towards 440/I-287/Woodbridge/Perth Amboy
• Follow signs for Route 9 South/NJ-440 S/I-287/The Amboys. Merge onto Route 9 South
• Continue straight onto Smith Street/656, slight right to stay on Smith Street.
• Turn Right onto Wilson Street. Our health center is on the corner of Wilson Street and Market Street.
DIRECCIONES A LA CLINICA DE PERTH AMBOY
Desde el NJ Turnpike Norte:
• Salida 10-Cuandao pague quedese a la izquierda, sig alas senales que dicen Perth Amboy/Staten Island/440. Quedese en la linea derecha de la 440 en direccion a Perth Amboy.
• Tome la primera salida Smith Street y siga las senales para Smith St. La primera luz despues de la interseccion de Smith /Convery Blvd (Ruta 35) es Herbert St.
• Haga una derecha en Herbert ST. Esta calle hace una curva y se convierte en Market St.
• El 450 Market St esta en la esquina de Market y Wilson.
Desde La Ruta 287 Sur:
• Sig alas senales que dicen “Bussines District”/Smith St Exit. Despues proceda como en las direcciones de arriba.
Ruta 1:
• Norte en direccion a la 514 East/Woodbridge/Raritan, pase el Middlesex County College y el Raritan Civic Center, sig alas senales que dicen “Perth Amboy/440”, la primera salida es Smith Street.
• Despues que salga en esta salida sig alas dircciones de arriba.
Garden State Parkway:
• Siga el parkway hasta la Ruta 35 Norte.
• Vaya por arriba del Puente y haga una derecha en la primera luz.
• Esta calle es Smith St.

From Southern New Jersey
• Take Garden State Parkway North (local) to exit 109 (Route 520). Bear right on exit ramp (do not cross through traffic light –stay right). You will merge onto Newman Springs Road/Route 520 heading east. We are approximately 1 ½ miles from the Parkway exit.
• **After crossing railroad tracks, Spirits Liquors will be on your left. Planned Parenthood is directly across the street from Spirits Liquors. Our driveway is immediately after LaRosa’s Pastry Shop on right. Parking is available in the rear of the building.
From Northern New Jersey
• Take New Jersey Turnpike South to Exit 11.
• Take Garden State Parkway south (local) to exit 109 (520 East).
• Go through tollbooths to traffic light. Make left onto Newman Springs Road/Route 520. Proceed east approximately 1 ½ miles from Parkway exit.
• See second paragraph above (**).
From Route 18 (New Brunswick)
• Route 18 South to Route 520 (Tennant/Red Bank exit). Make left at stop sign onto 520 East.
• Take 520 approximately 13 miles and continue past Garden State Parkway.

DRIVING DIRECTIONS TO SPOTSWOOD HEALTH CENTER
From Route 18 South
• Take route 18 South to Rues Lane (see Macy’s) make right turn.
• Go to first traffic light, make left (Summerhill Road).
• Take Summerhill Road to the end.
• Make a right onto Main Street.
• Continue through 2nd light. Bear right at fork in the road onto Snowhill Street.
• Make immediate right into Snowhill Plaza. Parking in the rear.
DIRECCIONES PARA LA CLINICA DE SPOTSWOOD
Desde La Ruta 18 Sur:
• Toma la Ruta 18 Sur has la Rues Lane (a tu derecha vas a verla tienda Macy’s ), has una derecha en la Rues lane.
• En la primera luz (Summerhill Road) has una izquierda.
• Sigue en Summerhill Road hasta que termine.
• Cuando llegues al final haces una derecha en Main St.
• Vas a pasar dos luces y vas a ver como la calle se divide, quedate a mano derecha.
• Esta calle es Snowhill Street. Dobla derecha en el Snowhill Plaza estamos localizados alli.
